What Is Kamagra Soft? — Mechanism of Action
Sildenafil citrate is a selective PDE5 (phosphodiesterase type 5) inhibitor (Ki = 3.9 nM). During sexual stimulation, nitric oxide (NO) is released in the corpus cavernosum of the penis, activating guanylate cyclase and elevating cyclic GMP (cGMP). Elevated cGMP relaxes the smooth muscle of the cavernosal arteries, increasing arterial inflow and producing an erection.
PDE5 normally degrades cGMP, terminating the erection. Sildenafil competitively inhibits PDE5, preventing cGMP breakdown and sustaining the vasodilatory response. The result is a firm and lasting erection in the presence of sexual stimulation. Sexual arousal is essential — sildenafil does not produce spontaneous erections.
Sildenafil also inhibits PDE6 (expressed in retinal photoreceptors) at higher plasma concentrations, which is the pharmacological basis for the mild, transient visual side effects (blue tinge, photophobia) occasionally reported with sildenafil but not with tadalafil or vardenafil.
Dosage and How to Take
- Chew one 100mg tablet thoroughly before swallowing — do not swallow whole
- Take 20–30 minutes before anticipated sexual activity; some men may need up to 60 minutes
- Do not exceed one 100mg tablet per 24-hour period
- No water is required, but a small amount may assist swallowing after chewing
- For best results, take on an empty stomach or after a light meal only — high-fat meals delay onset by up to 60 minutes and reduce peak concentration by ~29%

Critical Safety Information — Nitrates Interaction
Note — nitrate window vs tadalafil: The mandatory safety gap for sildenafil is 24 hours. For tadalafil (Cialis, Cialis Super Active), the required gap is longer at 48 hours, due to tadalafil's longer half-life of ~17.5 hours vs sildenafil's 3–5 hours. If you take any nitrate medication regularly for angina or heart disease, consult your GP or cardiologist before using any PDE5 inhibitor.
Also absolutely contraindicated with: Riociguat (Adempas). Combination with alpha-blockers (tamsulosin, doxazosin, alfuzosin) may cause symptomatic hypotension — use with caution and only under medical guidance.

- Sudden loss or significant decrease in vision — possibly associated with non-arteritic anterior ischaemic optic neuropathy (NAION). Risk is higher in men with pre-existing vascular risk factors
- Sudden decrease or loss of hearing, with or without tinnitus or dizziness
- Severe hypotension — especially if taken with nitrates, alpha-blockers or large amounts of alcohol
Note on visual side effects: Sildenafil inhibits PDE6 in addition to PDE5. PDE6 is expressed in retinal rod photoreceptors and plays a role in phototransduction. Inhibition at higher plasma concentrations produces mild, reversible visual changes — a bluish tinge to vision (cyanopsia), photophobia or mild blurring — typically within 1–2 hours of dosing and resolving within 4 hours. These effects are not seen with tadalafil or vardenafil, which are more selective for PDE5. If visual disturbances are prolonged or severe, discontinue use and seek medical advice.
